Nearly 1 in 3 Ragdolls Carry a Heart Gene That Can Cause Sudden Death at 15 Months. Here’s the Test Most Owners Don’t Know to Ask For.

Your Ragdoll can pass every vet checkup looking perfectly healthy and still be carrying a gene that can stop their heart without warning.

Nearly 1 in 3 Ragdolls carries a genetic mutation called MYBPC3 R820W — a single change in their DNA that causes the walls of the heart muscle to slowly thicken until the heart can no longer pump effectively. The condition is called hypertrophic cardiomyopathy, or HCM. In Ragdolls who inherit two copies of the mutation, it can trigger sudden cardiac death as early as 15 months old.

Most Ragdoll owners have never heard of it. Not because it’s rare — studies show between 17% and 30% of the breed carry the gene. Because HCM is quiet. A cat can sit in your lap, eat full meals, play with a feather wand, and seem entirely normal while the walls of their heart grow thicker with every beat.

Rocky’s owner thought the same thing. The 7-year-old Ragdoll was his usual self right up until the morning his sides were heaving. By then, fluid had filled his lungs. He spent 48 hours in an oxygen tent at the emergency vet before he could breathe on his own. His owner did palliative care from home and lost him in early February.

Why a Normal Checkup Won’t Catch It

A standard vet exam includes a stethoscope listen to the heart. For many cats, that’s enough. For Ragdolls, it often isn’t. HCM in early stages may produce no murmur at all — or one so faint it blends in with normal variation. A clear listen doesn’t mean a clear heart.

Annie was diagnosed with HCM at seven after five months of low-level symptoms: slightly lethargic, off her food, slow on the stairs. Her regular vet didn’t catch it. Neither did the feline internist. It took a veterinary cardiologist and an echocardiogram to finally name it.

Signs Worth Watching For

The warning signs are subtle by design. Cats evolved to conceal weakness, and HCM gives them a lot to conceal. Watch specifically for:

  • Resting respiratory rate above 30 breaths per minute — count for 30 seconds while your cat sleeps deeply, double it, and track it over time
  • Panting without heat or physical exertion
  • Front legs set wide with elbows angled out, as if making more room in the chest
  • Sudden drops in energy in a cat who normally follows you room to room
  • Reluctance to jump or climb stairs without a clear reason
  • Loss of appetite with no other obvious cause

None of these are definitive on their own. All of them warrant a call to your vet.

Two Ways to Get Ahead of This

The first is a genetic test — a cheek swab available through the UC Davis Veterinary Genetics Laboratory or through your vet. It tells you how many copies of the MYBPC3 R820W mutation your cat carries. One copy (heterozygous) carries a lower risk; many of these cats live a normal lifespan. Two copies (homozygous) significantly raises the risk of early, severe disease. The test doesn’t confirm whether HCM has developed — but it tells you whether the risk is there.

The second — and more complete — step is an echocardiogram: a heart ultrasound performed by a veterinary cardiologist. This is the only way to definitively diagnose HCM. According to the Cornell University College of Veterinary Medicine, cats at risk for HCM should receive cardiology screening at least once a year. For Ragdolls, that recommendation applies to the whole breed.

When to Go Right Now

Head to an emergency clinic immediately — not in the morning, right now — if your Ragdoll is breathing open-mouthed, breathing more than 40 times per minute, sitting hunched with their sides visibly heaving, or showing sudden weakness or paralysis in the hind legs. That last sign can indicate a thromboembolism: a blood clot that has left the heart and blocked circulation to the limbs. It is one of HCM’s most sudden and serious complications.

What Responsible Breeders Do Differently

If you’re considering adding a Ragdoll kitten to your home, ask the breeder directly: have both parents been tested for the MYBPC3 R820W mutation? Ethical breeders test for HCM and do not breed positive cats. According to NC State Veterinary Hospital, removing genetically positive cats from breeding programs is the primary way the community works to reduce HCM prevalence over time. A breeder who can’t answer that question is worth walking away from.

It’s Not a Death Sentence — But You Have to Know First

Carrying the gene is not a death sentence. Many Ragdolls with one copy live full, healthy lives. Cats diagnosed early — before fluid builds, before crisis — can be monitored, managed with medication, and given years they wouldn’t otherwise have had.

The difference is almost always whether the owner knew to look.

Rocky’s owner wishes they had asked sooner. Annie’s owner, who spent five months watching a cat quietly decline without knowing why, says the same thing. Ask your vet about HCM screening at your next visit. It’s a swab or an ultrasound. Your Ragdoll has already done harder things — like tolerating you picking them up like a baby every single morning.
